Participation in our services does not create a traditional in-person doctor-patient relationship.

3. Intake Application & Qualification Requirement

Before any appointment, consultation, or treatment discussion can occur, patients must:

• Provide accurate and complete health information
• Submit payment for the medical review fee (if applicable)

A licensed provider will review your submitted information to determine whether treatment is appropriate. Completion of the intake application does not guarantee qualification or approval for treatment.

Fees cover the time, expertise, and administrative costs associated with reviewing your medical information and coordinating care. Refunds will not be issued if you do not qualify for treatment, choose not to proceed, or discontinue services.

6. Prescription Medications

Prescription medications are provided only when deemed medically appropriate by a licensed provider. Shape U Health does not guarantee access to any specific medication, dosage, or treatment plan.

Medication availability may be subject to pharmacy supply, regulatory requirements, and provider discretion.

7. Patient Responsibilities

You agree to:
